Care Coordinator-Oneonta

Catholic Charities Care Coordination Services seeks a Care Coordinator to provide core services to individuals with chronic illnesses enrolled in NY’s Health Homes. Care Coordinators assist individuals access the services necessary to obtain positive health outcomes, reduce emergency room and inpatient stays, maintain their quality of life, and promote independent functioning. The key to this service model is to coordinate and communicate with an inter-disciplinary team serving each individual.
Typical duties and responsibilities include:
• outreach & engagement
• intake and obtaining participant consent
• assessment, care plan development and implementation
• monitoring and follow-up
• crisis intervention
• advocacy
• interagency and intra-agency coordination.
Qualifications: 60 credit hours of college study from a regionally accredited college or university and two years of relevant* case management; or Bachelor’s Degree in health, education or human services or RN and one year of relevant* case management experience; or Associate’s Degree and three years of relevant* case management experience; or LPN or certification as a CASAC and two years of relevant* case management experience
*Relevant case management includes working with individuals with HIV/AIDS or other chronic conditions; a history of mental illness; homelessness; chemical dependence; and/or other populations of persons in need. Knowledge of behavioral health and other chronic illnesses preferred.
Experience and Required Skills:
Familiarity with and sensitivity to specialized needs of people with HIV and other chronic illnesses, of people with substance abuse and mental health issues, of people of color, of gay, lesbian, bisexual and transgender individuals. Ability to work with people of diverse backgrounds preferred.
Knowledge of multiple chronic illnesses, including HIV/AIDS, preferred.
Familiarity with the cycle of poverty and working with families experiencing poverty.
Familiarity with and ability to access community resources and services.
• Strong communication skills, including verbal ability, writing style and active listening skills.
• Computer literacy and demonstrated ability to work within a windows electronic health record format including competency in using laptops
• Attention to detail.
• Strong organizational skills including time management and ability to prioritize.
• Ability to work as a member of a team.
• Ability to provide strengths-based, client-centered, proactive care coordination.
• Ability to read/comprehend written and verbal instructions.
• Ability to manage stress appropriately, make decisions under pressure, handle multiple priorities, work alone and manage anger/fear/hostility/violence of others appropriately.
• Ability to interact with people from diverse backgrounds and provide an atmosphere of trust, mutual respect and caring.
• Ability to read and write in English
• Bi-lingual English/Spanish desired

Community Health Worker-Saratoga
Catholic Charities Care Coordination seeks a full-time Community Health Worker. This position provides outreach, engagement and supportive care coordination activities to eligible clients enrolled in Health Homes programs to access needed services necessary to maintain their quality of life, promote independent functioning and meet the outcomes of the Health Home model.
Minimum Qualification:
• Direct experience with or knowledge of population or community to be served.
• Excellent oral communication skills
• High level of comfort and ability to conduct outreach in neighborhoods throughout Saratoga Counties.
